"This report deals with an investigation ... concerning the introduction of contraband into the High Risk Management Unit (HRMU) of the Goulburn Correctional Centre ... On 14 March 2003, a search of two adjoining HRMU cells by correctional officers located a number of contraband items hidden behind the cells' wall light units. The items found were a number of mobile phones and Subscriber Identity Module (SIM) cards, a mobile phone battery charger, a miniature digital camera that connects to mobile phones, and a miniature ratchet device ... Preliminary inquiries by Commission officers indicated that a correctional officer, Cale David Urosevic, may have been involved ... [The] investigation ... also focussed on the relevant systems and procedures in place to prevent such conduct."--Executive summary.
