Sometimes attributed to Thomas Middleton and to Wentworth Smith; also historically misattributed to William Shakespeare, presumably because of the initials, and included in the third folio of 1664.

Boston Public Library (Rare Books Department) copy bound by Charles Lewis in red morocco panelled in double-ruled gilt. The title page (A2) is a facsimile and H4 has been repaired/restored. The outer formes of sheets B and D appear in their corrected states. The edges have been trimmed closely. The armorial bookplate of the Barton Library is present on the front pastedown.
