Vol. 19 is missing pp. 521-528; a loose insert of pages has been provided, possibly from another edition as the page numbers are 374-381.

Provenance : Vol. 1 (and other volumes still in original binding) stamped : 'Medical Research Council Library' and stamped 'Cancelled' ; Label pasted to Vol. 1 (and some of the volumes still in original binding) : 'Lent to Prof. Greenwood School of Hygiene' ; LSHTM Library accession stamp date - 7 Mar. 1973; most rebound copies have a bookbinder's label : 'Hand bound E.A. Weeks & Son London' and have additional LSHTM Library stamp date - 5 Feb. 1981.
