Title within a woodcut border (McKerrow, Borders, no. 55; Devices, no. 106) with Wyllyam Myddelton's mark in the shield at the bottom, supported by cherubs. The device passed to Powell on Myddelton's death in 1547.
With unsigned preface by Richard Taverner, who was probably also the editor --cf. STC (2nd ed.) 9291.
Colophon: Imprynted at London in Fletestrte [sic] at the signe of the George next to Saynte Dunstones churche by Wyllyam Powell. In the yere of our lorde God. M.D.L.I. The .xix. daye of Marche.

Bookplate of George Dunn, who purchased it in July 1886, with his note on front pastedown. Signature at head of title page: 'Tho. Martin. 1720'
