Spaces left for initials; one large initial on leaf [4a] supplied in black ink.

Imperfect copy: leaves [6-7] of text wanting. In binding, an extra blank leaf has been inserted between the first two prelim. leaves (table and register) and original blank leaf [3]

Corner of first leaf cut away; some leaves foxed and stained.

A few manuscript notations in margins.

Bound in vellum.
