A reissue, with added general title page and conjugate blank, of "A forme of wholsome words" (printed by John Raworth), "The righteous mans plea to true happinesse" (printed by Thomas Cotes, Richard Badger and the assigns of Thomas Purfoot), and "A learned treatise" (printed by Richard Hodgkinson). The first and third works were edited by Anthony Burgess. In the second section, "The fifth sermon" (caption title) begins new pagination.

The words "the definition and distribution of" are enclosed in brackets on the title page.
