About the Book

To investigate a means of reducing the risk of acute wound healing complications associated with preoperative irradiation, we compared the original treatment plans of 24 patients with extremity STS to conformal and intensity-modulated radiotherapy (IMRT) plans to determine the amount of dose reduction possible to the future surgical flaps, without compromising target coverage. For mean percentage of flap treated to 30 Gy or more, there was no significant difference between the original and conformal plans, but there was between the original and IMRT plans (p=0.0005) and the conformal and IMRT plans (p=0.0002). There was no difference in the mean bone or CTV dose between the 3 plans, but there was a significant difference in mean dose to flaps for all 3 plans (p=0.0001). There was no difference in mean target coverage between all three plans. There was improved target conformality when employing IMRT as compared to the original and conformal plans (p=0.0001).
