Published 1696 by London, printed by Charles Bill, and the executrix of Thomas Newcomb, deceas'd; printers to the kings most excellent majesty. Reprinted at Boston in New England by Bartholomew Green & John Allen, printers to the governour & Council. in [Boston] .
