The only known copy, held by the American Antiquarian Society, is mutilated; missing portion of title supplied from other editions. Cf. Evans 16097-16104.

Text in four columns with final sentences of address, resolve, and imprint continued vertically in right margin; surviving printed area measures 38.8 x 30.9 cm.
